1930 - 2014 Jack T. Ulsh, 84, of Middletown, entered into eternal res
t at home on Tuesday November 11, 2014 surrounded by his loving family
.
He was born on May 5, 1930 in Millerstown, Pa. to the late Miles O. an
d Minnie A. Troutman Ulsh.
Jack was a former bank manager for Commonwealth National in Middletown
, and a former sales consultant for Brenner and Titus Cadillac, forme
member of the Middletown Rotary Club, and former member of the Harri
sburg Masonic Lodge, and a former Middletown School Board member. Also
, he was an avid golfer and a diehard Philadelphia sports fan.
In addition to his parents, he was preceded in death by his first wife
, Thelma Ulsh and his second wife Nancy Ulsh, daughter Bonnie Crump, b
other Richard O. Ulsh, daughter in law, Denise Ulsh;
He is survived by his loving sons Barry R. Ulsh, Sr., of Middletown; B
ian K. Ulsh of Harrisburg; stepdaughter Tamara Reed of Middletown; gr
andchildren Stefanie Ulsh, Barry R. Ulsh Jr., Melissa, Jeremy, and Bra
dley Crump; ten great-grandchildren and two nieces.
Services for Jack will be held on Saturday November 15, 2014 at 11:00a
.m. at the Matinchek and Daughter Funeral Home and Cremation Services
, Inc., 260 E. Main St. Middletown. A viewing will be held from 10:00a
.m. until the time of services. Burial will follow the service at th
e Middletown Cemetery.

isburg, 7790 Grayson Road, Harrisburg, Pa. 17111.
The family has entrusted the care of the Matinchek and Daughter Funera
l Home to handle the arrangements.
